Purpose
Complete this application if you are an employer who is applying for a Work-Sharing Agreement. You will also have to complete the form EMP 5101 'Work-Sharing Unit Attachment "A"'. Please send the forms (EMP5100/EMP5101) electronically by email to the appropriate regional general delivery box:
