Complete this form if you are a sponsor/employer who wishes to submit a complete proposal. The cash flow forecast is used to demonstrate in which periods the contribution recipient requires funding. The cash flow forecast should reflect only the monthly breakdown of the forecast and should not include detail by cost item or cost category.
