Complete this form if you wish to have your Employment Insurance (EI) payment deposited directly to your bank account. You can mail or deliver this form to your local Service Canada Centre. Please be sure that you have completed sections "A," "B," and "C". Take extra care in completing the section "Financial Institution Information". Finally, be sure to sign the form in section "D".
