Consent to Deduction and Payment - Canada Pension Plan - ISP1613
Purpose
ISP has agreements with eight (8) Provinces (Newfoundland, Prince Edward Island, Nova Scotia, New Brunswick, Ontario, Manitoba, Alberta and British Columbia) which allow, with client's consent, reimbursement of retroactive payments when their application for CPP benefits has been approved. This form is applicable only to Provinces which have an agreement with CPP. Provinces or Territories that would like to have more information may call 1 800 277-9914.
