Terminal Illness Application for Disability Benefits under the Canada Pension Plan (ISP-2530A): Complete the application, sign it and return it by mail. 2. Terminal Illness Medical Attestation for a Disability Benefits Application under the Canada Pension Plan (ISP-2530B): Complete sections 1 and 2 and ask your physician or nurse practitioner to complete section 3, sign the form and return it by mail.
