Purpose
This form is to be completed by the Vocational Rehabilitation Service Provider and must be submitted to the Service Canada Vocational Rehabilitation Case Manager within 30 days of vocational rehabilitation services being terminated. Note: all outstanding invoices must be submitted with this report.
