Australia/Canada Agreement – Application kit for CPP Disability Benefits - ISP5053-AUS
Purpose
For persons residing in Australia to apply for a Canada Pension Plan Disability Benefit under the Agreement on Social Security between Canada and Australia. The Disability kit includes: Australia/Canada Agreement - Application for CPP Disability Benefits (ISP5053-AUS), Reference Guide and Medical Report for a Canada Pension Plan Disability Benefit (ISP2519-IO-AUS). Each form must be completed and forwarded to your nearest social security office (see Returning the Form).
