Morocco/Canada Convention - Application for CPP Disability Benefits - ISP5053-MAR
Purpose
For persons residing in the Kingdom of Morocco to apply for Canada Pension Plan Disability Benefits under the Agreement on Social Security between Canada and Morocco. The Disability kit includes: Application form, General Information and Guide, Questionnaire, 2 Consent for Service Canada to Obtain Personal Information forms and Medical Report. Each form must be completed and forwarded to your nearest Service Canada office (see Returning the Form).
