Title EMPLOYER’S ANNUAL HAZARDOUS OCCURRENCE REPORT (EAHOR) (Onboard)
Purpose Federally regulated employers with onboard activity must complete this report on or before March 1 of each year. This annual report records the total number of hazardous incidents occurring in the workplace in the previous calendar year. The following information must be provided for each workplace:
  • Workplace information
  • Physical address of workplace
  • Injury data
  • Employment data

The EAHOR can be submitted either by email, Data Gateway, mail or by the ‘Submit’ button located on the PDF form.
