SUPPLEMENTAL UNEMPLOYMENT BENEFIT (SUB) PLAN REGISTRATION - NAS5036
Purpose
This form will be used by employers to register their SUB plan with Service Canada. Before any supplemental payments are paid to an employee who is in receipt of EI benefits during a period of unemployment due to temporary stoppage of work, training, illness, injury or quarantine, the employer must receive the approval from Service Canada that all the required conditions are met.
